MEDICAL PLAN
Assured Decisions, LLC offers a comprehensive health insurance plan, which provides employees and their covered dependents with the choice between two major medical and prescription plans along with a dental and vision plan through United Healthcare.

Assured Decisions, LLC pays the majority of the premium for the employee's chosen coverage and provides an additional discount by deducting the employee's per pay contributions from their paycheck on a pre-tax basis.

LIFE INSURANCE/ ACCIDENTAL DEATH & DISMEMBERMENT
Assured Decisions, LLC provides two-times salary coverage life insurance through Lincoln Financial Group to a maximum of $350,000 at no cost to its employees. Assured Decisions, LLC also provides employees with Accidental Death & Dismemberment insurance at two-times salary to a maximum of $350,000 at no cost to the employee, which covers against accidental death or loss of limb, hearing or sight.

DISABILITY PROGRAMS
Assured Decisions, LLC offers a two-step disability program through Lincoln Financial Group:

Short-Term Disability (STD)
66.67% of weekly salary up to $1000 per week. Benefits begin on the 1st day for accident and the 8th day for sickness. The maximum benefit period is 13 weeks and is tax-free.

Long-Term Disability (LTD)
60% of monthly salary up to $7000 per month. Benefit starts at 90 days and is tax-free.

Premiums for these coverages are deducted from employees' paychecks per pay, thereby providing a tax-free benefit.

FLEXIBLE SPENDING ACCOUNTS
Assured Decisions, LLC offers Flexible Spending Accounts (FSA) for health care and dependent care with a convenient debit card through CBIZ Benefits and Insurance. FSA's provide employees with a way to pay for allowable expenses in tax-free dollars.

LEAVE PROGRAMS
Assured Decisions, LLC employees begin accruing paid time off (PTO) immediately after their date of hire and accrue an additional day each year on their hiring anniversary until they reach 25 days. Employees also have the opportunity to "cash out" a portion of their PTO balance once a year.

In addition, the Company provides paid leave for Federal holidays according to the Government holiday schedule; other types of leave include bereavement, jury duty, and military.

401K SAVINGS PLAN
Assured Decisions, LLC offers the ability for employees to participate in the Company’s 401(k) plan. Employees may elect to contribute a portion of their base salary (subject to IRS restrictions) to a Traditional (pre-tax) or Roth (after-tax) plan. The plan is comprised of over 25 investment vehicles that employees may choose to allocate their funds.

The company contributes a match of .25/$1.00 up to 6% salary to augment employees' contributions which applies to a three-year vesting schedule. Employees age 50 and over are eligible to participate in a "catch-up" provision and contribute extra money up to the IRS maximum.
