Assured Decisions, LLC offers a comprehensive medical plan, which provides employees and
their covered dependents with major medical inpatient and outpatient services, vision, dental,
prescription drug, and wellness care.
Employees have the option of employee only, employee plus child, employee plus spouse, or family
coverage. Assured Decisions, LLC pays the majority of the premiums for chosen coverage by the employee.
| LIFE INSURANCE |

Assured Decisions, LLC provides two-time salary coverage life insurance to a maximum of $350,000 at no cost to
its employees. For coverage above the company provided life insurance,
employees may purchase additional coverage for themselves and family members.
| ACCIDENTAL DEATH & DISMEMBERMENT |
 |
Assured Decisions, LLC provides employees Accidental Death & Dismemberment insurance, which covers against
accidental death or loss of limb, hearing or sight. The benefit amount is also 2 times salary to a maximum of $350,000 and is provided at no cost to the employee.
| DISABILITY PROGRAMS |

Assured Decisions, LLC offers a two-step disability program:
Short-Term Disability (STD)
66.67% of weekly salary up to $1000 per week. Benefits begin on the 1st day for accident and the 8th day for sickness.
The maximum benefit period is 13 weeks and is provided at no
cost to the employee.
Long-Term Disability (LTD)
60% of monthly salary up to $7000 per month. Benefit starts at 90 days and is provided at no cost to employees.

Assured Decisions, LLC offers Flexible Spending Accounts for health care and dependent day care.
Flex spending accounts provide employees a way to pay for allowable expenses in tax-free dollars.
| LEAVE PROGRAMS |
 |
Assured Decisions, LLC employees begin accruing paid time off (PTO) immediately after their
date of hire. In addition, the Company celebrates Federal holidays according to the Government holiday schedule. Other
leave programs include bereavement, jury duty, and military leave.
| 401K SAVINGS PLAN |

Assured Decisions, LLC offers the ability for all full time employees working at least 1,000 hours per year to participate in the
Companys 401(k) plan. Employees may elect to contribute a
portion of their base salary (subject to IRS restrictions) to a Traditional (pre-tax) or Roth (after-tax) plan. The company makes an annual
contribution to augment the employees' contributions.
A three year vesting schedule applies to the employer match. Employees age 50 and over are eligible to
participate in a catch-up provision and contribute extra money up to the IRS maximum. The plan is comprised
of over 25 investment vehicles that employees may choose to allocate their funds.

Assured Decisions, LLC provides employees the opportunity to receive technical/job-related
training through our Staff Development Program. This program includes educational
assistance, professional/technical society memberships and various training programs.
